we accept most major dental insurance plans (please see the list of examples below). don’t see your insurance listed? rest easy. call or text us at 970-966-2268 and our knowledgeable team members will be happy to assist you in determining your coverage and eligibility. at this time, we are not in network with medicaid.

as a courtesy to our patients, we always perform insurance verifications to help maximize dental benefits and minimize out-of-pocket costs. we highly encourage you to provide us with your insurance information as soon as possible so we can complete your complementary insurance verification in a timely manner.

insurance coverage will not dictate the level of care provided at maven smile design. at your appointment, dr. ralston will perform a thorough examination and explain different treatment options and costs to help you make the most informed decision.

before treatment is performed, we will discuss your treatment plan and financial options. this will allow you to fully understand the recommended treatment, what to anticipate in fees, and allow you time to make the necessary financial arrangements. financing options are available through sunbit (more information below). as another courtesy, we will file insurance claims on your behalf, saving you the time and hassle.
